Comparison of Robot Assisted Oblique Lateral Interbody Fusion and Internal Fixation in Single Position and Reposition Surgery

Objective
To study clinical effect of robot-assisted oblique lateral interbody fusion (RAOLIF) and internal fixation in single position or reposition surgery.
Methods
Twelve patients underwent single position RAOLIF surgery operated by the same surgeon from July 2018 to September 2021 were retrospectively collected.The patients included four males and eight females aging (59.17±8.72) years.Twenty-four patients with matched age,gender,body mass index (BMI) and surgical segment were included in robot-reposition RAOLIF group.The patients were (59.50±7.76) years old.There were eight males and sixteen females.BMI,operation segments,operative time,intraoperative blood loss,times of radiations,length of postoperative hospital stay and unplanned secondary surgery were collected.Radiology parameters as disc height of upper segment,lumbar lordosis angle,foraminal height of surgical segment,facet joint invasions,screw deviation distance and screw breach grade were measured and analyzed.Screw acceptable rate was calculated.
Results
There was no difference between two groups in operation time,blood loss,postoperative hospital stay,disc height of upper segment,lumbar lordosis angle,and intervertebral foramen height (
P
>0.05).Number of radiation times in single position group was lower than reposition group (
P
=0.038).After surgery,foraminal height of surgical segment was increased in both groups (
P
<0.01).Screw accessibility rate was 98% in single group and 97% in reposition group.
Conclusion
RAOLIF surgery in single position has the same safety and accuracy as reposition surgery.It could reduce the surgical radiation and improve foraminal height,though could not decrease the operation time and blood loss.

Radiological Features and Clinical Findings in Degenerative Lumbar Scoliosis with Osteoporosis

Objective
To evaluate the radiological features and clinical findings in osteoporotic patients with degenerative lumbar scoliosis (DLS).
Methods
From November 2017 to December 2021,27 DLS patients combined with osteoporosis (OP) were enrolled as OP group.The OP group had 5 male and 22 femaile patients and had an average age of (68.1±7.4) years (51~79 years).At the same time,another 27 DLS patients without osteoporosis were recruited as the nonosteoporosis (NOP) group.The NOP group had 8 males and 19 females and had an average age of (66.3±6.9) years (53~76 years).Bone mineral density (T-score) and clinical symptoms were recorded.The radiographic parameters were measured in standing full spine X-ray film and further compared between two groups.The correlation between the T-score and radiographic parameters was analyzed in the OP group.
Results
The median T-score in the OP group (-3.0±0.6) was significantly lower than that in the NOP group (-0.6±1.4).The percentage of back pain,motor deficit and sensory deficit were comparable between two groups (

>0.05).There were 13 left lumbar curves and 14 right lumbar curves in the OP group.There were 11 left lumbar curves and 16 right lumbar curves in the NOP group.No significant differences were observed in terms of radiographic parameters including lumbar and thoracic cobb angle,apical vertebral translation,apical vertebral rotation,thoracic kyphosis,lumbar lordosis,coronal and sagittal trunk shift (
P
>0.05).A significant correlation was found between Tscore and thoracic kyphosis in OP group (r=-0.568,P=0.022).
Conclusion
The coronal and sagittal profile are comparable between DLS patients with and without osteoporosis.The severity of osteoporosis is obviously correlated with thoracic kyphosis in osteoporotic patients.

Influence of Diabetes Mellitus and Symptom Duration of Frozen Shoulder on the Prognosis of Surgical Treated Patient

Objective
To analyze and evaluate the influence of diabetes mellitus and symptom duration on the prognosis of frozen shoulder patients undergoing arthroscopic shoulder joint release.
Methods
A total of 45 frozen shoulder patients from June 2019 to April 2022 from the third affiliated hospital of Anhui Medical University were collected in this study,including 18 males and 27 females,aged 47~66 years,with an average of (53.4±6.1) years.All patients underwent arthroscopic surgeries after complete preoperative examination and perioperative preparation.The patients were divided into group A (within 3 months),group B (3~6 months),and group C (more than 6 months) according to the total duration of shoulder symptoms before surgery.This study analyzed the range of motion(ROM),disabilities of the arm,shoulder,and hand score(DASH),American shoulder and elbow surgeons(ASES),visual analogue score(VAS),pain duration,and total disease duration to explore the effect of diabetes mellitus and duration of frozen shoulder on the prognosis of patients with frozen shoulder treated by arthroscopic surgery.
Results
All patients were followed up for 1~19 months,with an average follow-up of(17.3±5.1) months.The degrees of shoulder flexion,abduction,and internal rotation of patients without or with diabetes after surgery were:(164.4±20.2)°、(169.9±18.7)°、(38.5±8.0)°,(161.3±19.8)°、(170.6±14.2)°、(34.6±9.4)°.The postoperative DASH score,ASES score,and VAS score of patients without or with diabetes were:(9.4±10.1)、(86.2±13.2)、(7.9±1.4),(12.5±12.9)、(82.6±17.1)、(7.8±1.9).Time to complete pain relief after the operation and the total duration of frozen shoulder in the patients without or with diabetes were (3.6±3.5) weeks、(31.4±7.3) months,(3.7±2.5) weeks、(38.7±12.5)months.The duration of shoulder symptoms of A、B、C group had a statistically significant effect on internal rotation (P=0.02),A、B、C group had significant differences in the total symptom duration and postoperative symptom duration (P<0.001),the ASES score of patients in group B and group C after surgery was significantly higher than that in group A (P=0.02),and the DASH score of patients in diabetic group was significantly higher than that in non-diabetic group (P=0.03).
Conclusion
Arthroscopic release surgery is an effective method for the treatment of frozen shoulder,and its efficacy is not significantly related to the time of onset of the patient.However,diabetes has a significant impact on the postoperative recovery of patients.

Comparative Study of Dorsal Branch Chain Flaps of Digital Artery with Proximal and Middle Segments As Donor Sites in Repairing the Homodigital Fingertip Defects

Objective
To analyze and compare the effect of dorsal branch chain flaps of digital artery with proximal and middle segments as donor sites in repairing fingertip defects of the homodigital finger.
Methods
A retrospective analysis was performed on 48 patients (48 fingers) with defects of the 2nd to 5th fingertips admitted to the Second Hospital of Tangshan from February 2016 to August 2020.Dorsal branch chain of proximal phalangic artery flaps were used in 26 fingers of 26 cases,including 17 males and 9 females.The age ranged from 18 to 55 years,with a mean ago of (36.5±10.9) years.Dorsal branch chain of middle knuckle finger artery flaps were used in 22 fingers of 22 cases,including 14 males and 8 females.The age ranged from 20 to 54 years,with a mean ago of (35.8±10.7) years.Age,gender,wound size,operation time,follow-up time,skin flap appearance satisfaction,static two-point discrimination,total range of motion of injured knuckles and postoperative donor site appearance were compared between the two groups.
Results
All flaps survived successfully after operation.The follow-up time was 12 to 20 months.There were no significant differences in age,sex,wound size,operation time,follow-up time and total range of motion of injured digits between the two groups (
P
>0.05).The satisfaction of flap appearance,static two-point discrimination,and postoperative appearance of donor site of patients in the middle segment donor site group were better than those in the near segment donor site group,and the differences were statistically significant (
P
<0.05).
Conclusion
The dorsal branch-chain flaps of digital artery with the skin of the proximal and middle segments as the donor site is a simple operation with reliable blood supply.It is suitable for repairing soft tissue defects of the homodigital fingertip.Compared with the proximal donor site flap,the dorsal branch-chain flaps of digital artery with the middle segment as the donor site has more beautiful in appearance,less scar in the donor site,and better postoperative sensory recovery.

Clinical Effect of Zadek Osteotomy on Haglund Syndrome

Objective
To investigate the clinical effect of Zadek osteotomy (ZO) in the treatment of Haglund syndrome (HS).
Methods
A retrospective study was conducted on 17 patients who underwent ZO treatment for HS in the Department of Foot and Ankle Surgery of the Honghui Hospital affiliated to Xi'an Jiaotong University from March 2018 to July 2020 and were finally followed up.11 males and 6 females were included.The age ranged from 23 to 58 years,with an average of (39.7±10.2) years.There were 14 left feet and 3 right feet.The American orthopaedic foot and ankle surgery(AOFAS) ankleposterior foot score and visual analog scale(VAS) were used to evaluate the improvement of foot function and pain before and at the last followup.Measure the range of motion(ROM) of the ankle joint before and after the operation of the affected foot and the ChauveauxLiet (CL) angle of the lateral Xray film of the affected foot.
Results
No deep infection or osteomyelitis occurred.17 patients were followed up for 12 to 24 months,with an average of (19.3±1.2) months.At the last followup,the AOFAS score increased from (51.2±5.1) to (88.4±4.5),the VAS score improved from (6.4±2.3) to (0.9±1.0),the ankle dorsiflexion activity increased from (23.4±2.4)° to (29.3±1.2)°,the plantar flexion activity changed from (32.4±3.4)° to (33.1±3.5)°,and the CL angle changed from (18.1±1.2)° to (9.3±2.0)°.
Conclusion
Using ZO to treat HS patients can significantly improve the pain symptoms and ankle function,and slow down the progress of Achilles tendon disease,which is a safe and effective technology.

Study on The Relationship Between Serum Cartilage Oligomeric Matrix Protein and Knee Osteoarthritis

Objective
To analyze the relationship between serum cartilage oligomeric matrix protein (COMP) level and the occurrence and development of knee osteoarthritis (KOA).
Methods
255 patients with bilateral KOA admitted to our hospital from April 2012 to March 2019 were selected as the KOA group,and 100 healthy volunteers who came to our hospital for physical examination during the same period were selected as the control group.There were 168 males and 87 females in KOA group.The patients aged from 28 to 85 years old,and the average age was (59.77±11.01) years old.There were 67 males and 33 females in the control group.The patients aged from 20 to 86 years,and the average age was (58.96±12.42) years old.The level of serum COMP was detected by enzyme-linked immunosorbent assay.The Kellgren-Lawrence (K/L) scale,the Western Ontario and McMaster Universities(WOMAC) osteoarthritis index and Lequesne index were used to assess KOA disease severity.
Results
The serum COMP level in KOA group was significantly higher than that in control group (
P
<0.001).According to receiver operating characteristic analysis,the area under the curve of KOA at serum COMP level was 0.789.The probability of smoking history,duration of pain and the expression of typeⅢ procollagen amino terminal peptide (PⅢNP) in patients with serum COMP high-level group were higher than those in low serum COMP level group,and the Timed Up and Go test was lower than those in low serum COMP level group (
P
<0.05).Spearman rank correlation analysis showed that serum COMP level was positively correlated with WOMAC,Lequesne index and K/L grade in KOA patients (r=0.195,0.393,0.481,
P
<0.05).Multivariate logistic regression analysis showed that when serum COMP levels were between 2 210.59 to 3 143.87 ng/mL or >3 143.87 ng/mL,the risk of moderate and severe progression of K/L grade in patients with KOA increased by 2.091 times and 5.682 times,respectively (
P
<0.001).
Conclusion
Serum COMP level is generally increased in KOA patients,and high serum COMP level is also associated with increased disease severity,which is expected to be a potential biomarker for clinical diagnosis or assessment of disease progression.

The Relationship Between Psoas Muscle Index,Lumbar Vertebral Bone Marrow Fat Fraction and Bone Mineral Density and the Diagnostic Efficacy of Osteoporosis

Objective
To explore the relationship between lumbar psoas muscle index (PMI),lumbar vertebral bone marrow fat fraction (FF) and bone mineral density (BMD) and the diagnostic efficacy of osteoporosis (OP).
Methods
200 cases of BMD exam in our hospital from March 2018 to May 2021 were selected as the research objects.According to BMD,they were divided into normal bone mass group (T value≥-1.0,n=105) and bone mass reduction group (-2.5<T value <-1.0,n=57),OP group (T value ≤-2.5,n=38).General information and BMD,PMI,lumbar vertebral bone marrow FF were compared in 3 groups.The relationship between PMI,lumbar vertebral bone marrow FF and BMD were analyzed.OP’s Logistic regression model was established.The diagnostic efficacy of PMI and lumbar vertebral bone marrow FF for OP was evaluated.
Results
The BMD and PMI of the OP group were lower than those of the osteopenia group and the normal bone mass group,and the FF was higher than that of the osteopenia group and the normal bone mass group (
P
<0.05).The BMD and PMI of the osteopenia group were lower than those of the normal bone mass group,and the FF was higher than that of the normal bone mass group (
P
<0.05).PMI was positively correlated with BMD,and lumbar vertebral bone marrow FF was negatively correlated with BMD (
P
<0.05).Age,body mass index,PMI,lumbar vertebral bone marrow FF were the influencing factors of bone loss,and age,PMI,lumbar vertebral bone marrow FF were the influencing factors of OP (
P
<0.05).The AUC of the combined diagnosis of OP with lumbar vertebral bone marrow FF was 0.853,and it was greater than the single index diagnostic value (
P
<0.05).
Conclusion
PMI and lumbar vertebral bone marrow FF are independent influencing factors of BMD,and the combination of the two has a high efficiency in the diagnosis of OP.

Structural Evolution of Staphylococcus Aureus Biofilm on the Surface of Orthopedic Titanium Plate in Acute Pha

Objective
To observe the biofilm structure of Staphylococcus aureus on the surface of titanium plates at different time points during the acute phase (4 weeks) by laser confocal microscopy and scanning electron microscopy.
Methods
Staphylococcus aureus was cultured in vitro with orthopaedic titanium plates to form biofilms at different time points.After 1 week,2 weeks,3 weeks and 4 weeks of culture,the titanium plates were taken out,the biofilms points were stained with protein and propidium iodide dyes according to the different time points.After staining,the morphological structure of bacterial biofilms was observed by laser confocal microscope and scanning electron microscope.
Results
Laser confocal images and electron microscope images of biofilms at different time points were obtained.It was observed that a small amount of extracellular polysaccharide polymer was formed in bacteria at 1 week,and the internal spatial structure was disordered,which indicating the formation of early biofilms.With extension of the culture time,the amount of bacterial exopolysaccharide polymer gradually increased,the internal gaps contact with pores,and the spatial structure became complicated gradually.At 4 weeks,a large number of exopolysaccharide polymers were formed,and the inner membrane structure was wellstructured.It meant that a mature biofilm was formed.The number of viable bacteria in each time point had no difference(
P
>0.05),and the biofilm thickness between each time point had significant difference(
P
<0.05).
Conclusion
Staphylococcus aureus bacterial biofilm has difference between early and late phase.It is a dynamic evolution processfrom from early biofilm to late mature biofilm,which is manifested in the maturity and stability of bacterial biofilm exopolysaccharide polymers and internal spatial structures.
