同种异体骨软骨移植治疗关节软骨损伤的现状

摘要关节软骨是一种特殊的透明软骨组织，由细胞外基质与少量软骨细胞构成，关节软骨损伤在年轻、活跃的人群中非常常见。损伤范围较大、有明显症状的关节软骨损伤影响患者运动和日常生活活动，可致骨关节炎的发病率明显提高，由于关节软骨的再生能力及自我修复能力较差，使关节软骨损伤的自然愈合有限。同种异体骨软骨移植(osteochondral allograft transplantation，OCA)从组织捐赠者获得整块软骨和软骨下骨来精确修补受损的关节软骨，是一种针对局限性大块关节软骨损伤的有效治疗方式。与其他关节软骨损伤修复的手术方式如微创骨髓钻孔技术、自体软骨细胞植入、自体骨软骨移植等相比，OCA在中等范围(＞2 cm²)及大范围(4~10 cm²)软骨缺损的临床治疗中显示了更好的疗效，其组织来源较广泛，可根据健康关节软骨的外形、边界自由选择移植物尺寸，移植物存活不受损伤部位微骨折影响。此外与其他修复方式相比，接受OCA的患者术后可以更好地恢复活动与负重。这些优势使得OCA在中大型软骨损伤的临床实践中得到广泛的应用，在容易获得新鲜同种异体移植物的地区，OCA已经成为治疗局限性大块关节软骨损伤的有效手段，也成为了有运动需求的年轻患者的首选治疗方式。从20世纪80年代后，OCA逐渐主要针对关节软骨和骨局限性缺损进行治疗并取得了良好的疗效，最终应用于膝关节软骨缺损修复并拓展至髋、肩肘、足踝等关节。已发表的文献报告了5 290例膝关节OCA手术，成功率为83.2%。踝关节OCA为579例，成功率为73.8%；接受肩肘、髋关节OCA的患者有83例，结果也良好。OCA目前已在北美及欧洲地区广泛开展，形成了完整的组织移植体系并进行商业化发展。巴西参考上述体系并对其进行变革，建成了符合其本国特色的OCA组织移植体系，同时进行了大量的OCA移植验证了该体系的有效性。在我国OCA仅有零星的个案报道，距离在软骨损伤治疗中的广泛应用仍有较大差距。
