单侧双通道脊柱内镜治疗颈椎病的临床应用进展

摘要近年来，随着社会科技进步与生活方式改变，长时间以低头姿势伏案工作会引起颈椎椎间盘压力增大，加快颈椎退变，颈椎病发病率随之增高。临床上以神经根型颈椎病、脊髓型颈椎病多见，其症状明显影响日常生活和工作，患者经保守治疗无效后需行手术治疗。传统手术可以充分暴露术野进行彻底减压，需广泛剥离软组织，对组织破坏较大，容易造成术后肌源性颈痛不适、喉上神经及喉返神经损伤、切口血肿、食管瘘、硬脊膜撕裂等并发症，降低患者预后水平。随着脊柱内镜技术不断的发展以及患者对微创化、快康复的需求，单侧双通道内镜技术(unilateral biportal endoscopic discectomy，UBE)因工作通道、观察通道互不干扰，提高了术中操作效率，且在稳定光源下提供了清晰广阔术野，减压范围大，深受脊柱外科医生青睐。
当前，UBE已广泛用于治疗腰椎间盘突出、腰椎管狭窄、腰椎滑脱等腰椎退行性疾病，经过国内外脊柱外科医生不断探索及完善，UBE适应证逐步扩大，颈椎病也纳入其中。本文回顾国内外相关文献，总结其临床上的应用效果，旨在为UBE微创化治疗颈椎病提供新的思路，现综述如下。
