日间关节置换术的短期疗效研究

摘要目的本研究旨在评估西安市红会医院目前日间髋膝关节置换术临床路径的可行性与安全性。方法 2021年12月至2022年3月在日间关节置换术临床路径中共纳入了30例患者，其中男9例，女21例；年龄40~77岁，平均(66.03±7.97)岁。记录患者出入院时间，将出入院时间≤24 h定义为日间手术。出院后通过电话和问卷随访调查患者疼痛、恶心呕吐及不良事件发生率及其满意度。结果纳入路径的患者中28例获得随访，2例失访。患者满意度随访时间为术后7~42 d，平均(26.00±10.58)d。30例患者中，按时出院25例，未能按时出院5例。其中术后拍片延误2例，疼痛、头晕和患者拒绝出院各1例。出院后1 d有8例患者轻度疼痛(28.57%)，13例患者中度疼痛(46.43%)，3例患者重度疼痛(10.71%)；出院后7 d有14例患者轻度疼痛(50%)，13例患者中度疼痛(46.43%)。获得随访的患者非常满意18例(64.29%)，满意9例(32.14%)，中立1例(3.57%)。结论流程不完善、术后并发症和患者意愿是未能按时出院的主要原因，出院返家后无重大并发症，提示日间关节置换术是可行且安全的。
