同种异体骨软骨移植治疗软骨损伤的历史

摘要关节软骨是一种具有复杂结构的特殊组织，由细胞外基质及少量的软骨细胞构成，没有血管、淋巴管及神经支配。创伤、剥脱性骨软骨炎、关节软组织损伤、类风湿性关节炎等病因均可导致关节软骨损伤，可累及关节软骨和软骨下骨。由于软骨组织缺乏血管，再生能力较差，损伤后几乎无法自愈，如果不及时治疗，软骨损伤可能会导致关节肿胀和疼痛，最终发展为骨关节炎。关节软骨损伤在临床治疗中是极具挑战性的难题，在过去的几个世纪中几乎被认为是无法治疗的顽疾。William Hunter于1743年指出：从希波克拉底时期至今，软骨损伤一直是一种棘手的疾病，软骨一旦被破坏，就无法修复。从那时起，临床医生一直在努力寻找可靠的方法来修复关节软骨损伤。经历近4个世纪的研究与临床实践，目前已有多种技术被证明可以修复关节软骨损伤，其中同种异体骨软骨移植(osteochondral allograft transplantation，OCA)是治疗局限性大块关节软骨损伤的有效手段。OCA的发展主要经历了初步尝试阶段、探索研究阶段、应用推广阶段，至今OCA已在北美、欧洲、巴西等地区广泛开展，并为大量关节软骨损伤的患者带来康复机会。
