改良闭合复位张力带固定术治疗横行髌骨骨折的临床疗效

摘要目的探讨改良闭合复位张力带固定术治疗横行髌骨骨折的临床疗效。方法回顾性分析自2016年10月至2018年10月我院采用闭合复位张力带固定术治疗的63例横行髌骨骨折患者资料。对照组31例，采用常规闭合复位张力带固定术，男17例，女14例；年龄19~66岁，平均(38.5±13.6)岁；左侧10例，右侧21例；受伤至手术时间1~8 d，平均(4.1±2.1)d。改良组32例，采用改良闭合复位张力带固定术，男16例，女16例，年龄20~65岁，平均(40.9±17.2)岁；左侧11例，右侧21例；受伤至手术时间为2~7 d，平均(4.0±1.7)d。记录手术时间、骨折愈合时间、并发症发生情况及术后第3、6、12个月的美国特种外科医院(the hospital special surgery，HSS)膝关节功能评分和疼痛视觉模拟评分(visual analogue scale，VAS)。结果两组患者一般资料比较，差异均无统计学意义(P＞0.05)。两组患者均获完整随访，随访时间13~18个月，平均(15.5±1.5)个月。改良组手术时间较对照组明显减少，差异有统计学意义(P＜0.05)。两组患者术后切口均1期愈合。两组患者骨折均顺利骨性愈合，骨折愈合时间比较差异无统计学意义(P＞0.05)。均未出现骨不连、感染和内固定松动断裂等并发症。两组术后第3、6、12个月，改良组膝关节HSS评分均高于对照组，差异有统计学意义(P＜0.05)；改良组VAS均低于对照组，差异有统计学意义(P＜0.05)。结论与常规闭合复位张力带固定术相比，改良闭合复位张力带固定术治疗横行髌骨骨折，能够缩短手术时间，降低手术难度，在不影响骨折愈合情况下，能更好地改善膝关节功能及缓解术后疼痛。
