Abstract:Objective To explore the clinical effect of minimally invasive internal fixation under arthroscopy treatment on Schatzker type Ⅲ tibial plateau fracture.Methods 60 cases of Schatzker type Ⅲ tibial plateau fracture admitted to orthopedics department of our hospital from January 2016 to May 2019 were chosen and were divided into 2 groups according to the random table method,with 30 cases in each group.The treatment group was treated with minimally invasive internal fixation under arthroscopy,while the control group was treated with open reduction and internal fixation.The operative time,intraoperative blood loss,postoperative drainage volume,postoperative loading time,postoperative hospital stay,bone healing time and postoperative complication rate of the two groups were compared.Serum levels of interleukin(IL)-6,IL-1β,tumor necrosis factor (TNF) -α were compared between the two groups at 3d,6 months,and 12 months after surgery.The knee Lysholm function scores of the two groups were compared at 6 and 12 months after surgery.Results The intraoperative blood loss and postoperative drainage volume in the treatment group were lower than those in the control group (P<0.01),and the postoperative hospital stay,postoperative loading time and bone healing time in the treatment group were shorter than those in the control group (P<0.05,P<0.01).Serum levels of IL-6,IL-1β,TNF-α in the treatment group were lower than those in the control group at 3 days and 6 months after surgery (P<0.01).Pain,swelling and squatting scores in the treatment group were higher than those in the control group at 6 months after surgery (P<0.01).The incidence of shortterm and longterm complications in the treatment group was lower than that in the control group (P<0.05).Conclusion Minimally invasive internal fixation under arthroscopy can reduce intraoperative blood loss and postoperative drainage volume,and shorten postoperative hospital stay,postoperative loading time and fracture healing time,reduce the postoperative complications,lower levels of inflammatory factors,improve the function of knee joint,its treatment effect on Schatzker type Ⅲ tibia platform fracture is remarkable.
