富血小板血浆对胫骨高位截骨治疗膝骨关节炎的短期疗效研究

摘要目的探讨富血小板血浆(platelet rich plasma，PRP)对开放胫骨高位截骨(open wedge high tibial osteotomy，OWHTO)术后截骨区新生骨以及临床疗效的具体影响。方法采用前瞻性随机对照研究方法，选择2021年1月至2022年6月重庆大学附属三峡医院收治的19例(20膝)膝关节内侧间室骨关节炎患者，采用随机数表法将患者随机分为两组。其中PRP治疗组9例(10膝)，男4例(5膝)，女5例(5膝)；年龄44~65岁，平均(52.2±7.3)岁；空白对照组10例(10膝)，男5例(5膝)，女5例(5膝)；年龄33~68岁，平均(50.8±8.7)岁。所有患者均进行OWHTO，PRP治疗组于术中制备PRP凝胶并填充在胫骨开放截骨区域。术后利用放射治疗计划系统(radiation therapy planning system，TPS)monaco5.11进行胫骨开放截骨区域三维勾画并测量计算该区域平均CT值。比较两组患者一般临床资料、术前胫骨近端内侧角(medial proximal tibial angle，MPTA)，术前和术后12周疼痛视觉模拟评分(visual analogue scale，VAS)、西安大略和麦克马斯特大学(the Western Ontario and McMaster Universities，WOMAC)的骨关节炎指数，美国特种外科医院评分(hospital for special surgery，HSS)，以及术后1周、6周和12周胫骨开放截骨区域平均骨密度(CT值)。结果两组患者的一般临床资料(年龄、性别、身体质量指数)及术前MPTA比较差异均无统计学意义(P＞0.05)，具有可比性。所有患者均获得良好随访，两组患者术后12周VAS评分、WOMAC指数及HSS评分与术前相比均有明显改善，差异有统计学意义(P＜0.05)；术后12周PRP治疗组VAS评分、WOMAC指数及HSS评分较对照组改善更佳，但差异无统计学意义(P＞0.05)；两组患者术后胫骨开放截骨区域平均骨密度(CT值)随时间呈提升优化趋势，术后各时间点比较差异有统计学意义(P＜0.05)；在术后12周时PRP治疗组胫骨截骨区域平均骨密度(CT值)较空白对照组更高，差异有统计学意义(P＜0.05)。结论采用OWHTO治疗膝关节内侧间室骨关节炎，在术中对胫骨开放截骨区域使用PRP，对其术后截骨区域新生骨有明显生长促进作用，对临床疗效也有一定的促进和改善作用。
