新鲜前交叉韧带损伤伴前外侧韧带损伤的超声观察

摘要目的使用超声检查观察膝关节新鲜前交叉韧带(anterior cruciate ligament，ACL)损伤伴前外侧韧带(anterolateral ligament，ALL)损伤的发生率、损伤部位等特点。方法纳入2019年4月至2020年8月就诊于南京中医药大学附属医院运动医学中心的85例单侧膝关节新鲜ACL损伤患者，其中男74例，女11例；年龄16~48岁，平均(25.5±12.5)岁；左膝32例，右膝53例。均行患膝MRI检查和双侧膝关节超声检查，观察并记录健侧膝关节ALL的厚度、长度以及ALL胫骨止点与胫骨平台关节面、腓骨小头顶点、Gerdy结节的距离，同时观察并记录患侧膝关节ALL损伤的部位。结果 MRI检查：85例的患侧膝关节中仅61例显示有ALL这一结构，检出率71.76%。超声检查：85例的双侧膝关节均观察到ALL这一结构，检出率100%；85例的健侧膝关节显示，ALL在膝下外侧动脉上方水平的厚度平均为(1.3±0.1)mm，股骨起点至胫骨止点平均长度(39.1±3.7)mm，ALL胫骨止点与胫骨平台关节面距离(7.9±1.8)mm，ALL胫骨止点与腓骨头距离(21.3±2.7)mm，ALL胫骨止点与Gerdy结节距离(22.3±2.4)mm；85例的患侧膝关节中，有33例ACL损伤伴随ALL损伤，发生率38.82%，其中2例为Segond骨折，ALL损伤均发生在胫骨侧。结论膝关节新鲜ACL损伤伴发ALL损伤的发生率约为38.82%，超声检查是一种观察ALL损伤的简便、有效、无创的检查手段，且超声检查对ALL和ALL损伤的检出率高于MRI。
