Abstract:Objective To evaluate whether prolonged surgical duration increases the risk of postoperative knee function restriction in total knee arthroplasty (TKA) and to explore the influencing factors of surgical duration. Methods A retrospective analysis was conducted on 426 female patients with primary bilateral knee osteoarthritis who underwent first-time TKA at Peking University People’s Hospital from January 2017 to January 2019.The patients’ ages ranged from 50 to 82 years,with a mean age of (66.24±11.58) years.The surgical duration ranged from 28 to 150 minutes,with a mean of (64.03±19.17) minutes.The Knee Society Score (KSS) was used to assess postoperative knee function,with a score below 85 indicating postoperative functional limitation.Among the cases,405 had good knee function,while 21 had functional limitations.Univariate Cox regression analysis was used to assess the impact of surgical duration on postoperative knee function limitation.Further investigation was conducted to explore the relationship between surgical duration and postoperative knee function limitation,and multivariate logistic regression analysis was employed to identify the risk factors influencing surgical duration. Results The average surgical duration for the group with good postoperative knee function was (63.84±20.33) minutes,and the average age was (63.72± 13.92) years.In the group with functional limitations,the average surgical duration was (67.74±18.77) minutes,and the average age was (68.78±15.98) years.The results of the univariate Cox regression analysis showed that a surgical duration≥120 minutes (HR=1.814,95% CI:1.085~3.033) was a risk factor for postoperative knee function limitation.The multivariate regression analysis indicated that hypertension was an independent risk factor for increased surgical duration. Conclusion Prolonged surgical duration in TKA increases the risk of postoperative knee function restriction in patients,with hypertension being an independent risk factor for increased surgical duration.
