膝关节后外侧角合并前交叉韧带损伤治疗进展

摘要膝关节后外侧角(posterolateral corner，PLC)又称膝关节后外侧复合体(posterolateral complex，PLC)，是维持膝关节内翻、外旋稳定及限制胫骨后移的一组重要结构。膝关节PLC损伤因临床表现不典型，无特异性较高的专科查体方案，缺乏特异性影像学表征等问题，导致该疾病的早期诊断较为困难，被认为是膝关节的“暗区”。研究表明，在所有膝关节PLC损伤中，只有2%~28%是独立发生的，而临床上常见的膝关节PLC损伤常见于多发韧带损伤，尤其是合并前交叉韧带(anterior cruciate ligament，ACL)或后交叉韧带(posterior cruciate ligament，PCL)的损伤。最近的研究发现，PLC合并ACL损伤发病率远远被低估。本文将对PLC合并ACL损伤相关研究进行分析和讨论，对其生物力学及手术治疗进展做一综述，以期为PLC合并ACL损伤手术方案提供文献依据。
