半月板缝合术后医源性腘动脉损伤1例报告并文献复习

摘要随着关节镜技术和半月板缝合器械的不断发展，关节镜下半月板修复术已逐渐成为治疗半月板损伤的首选方法。常规的半月板缝合技术包括Inside-out、Outside-in和All-inside三种方法。膝关节镜外侧入路与外侧半月板后角以及腘动脉三者间存在特殊的解剖关系(见图1)，已有文献证明All-inside缝合技术缝合外侧半月板存在腘窝神经血管束损伤的风险，国外文献报道关节镜下外侧半月板后角缝合手术中，腘动脉损伤的概率为0.03%。据统计，腘动脉损伤是所有血管损伤中截肢率最高的类型。尽管半月板修复过程中神经血管损伤的发生率很低，但后果往往十分严重。一般认为当动脉损伤致肢体缺血超过6~12 h，治疗效果锐减。循环重建时间与患者预后密切相关，缺血6 h以内施行动脉重建手术的成功率约为90%，而24 h以后成功率仅为20%。现已有文献介绍了减少医源性腘动脉损伤的各种新技术，但如何避免腘动脉医源性损伤一直是具有挑战性的难题。2019年8月三峡大学附属仁和医院收治1例膝关节镜下All-inside技术修复外侧半月板后角撕裂导致医源性腘动脉损伤的病例，现报告如下。
