内侧髌股韧带重建术治疗复发性髌骨脱位的进展

摘要复发性髌骨脱位(recurrent patellar dislocation，RPD)是在第1次髌骨脱位后发生2次或2次以上脱位，或髌骨不稳的症状在第1次脱位后持续超过3个月。RPD病程越长引起的并发症越多，损害越严重。RPD可导致髌骨及股骨软骨损伤、膝关节滑膜肥厚、骨软骨游离体及骨关节炎等一系列并发症。同时，患肢的运动能力下降，可引起关节周围肌肉萎缩。内侧髌股韧带(medial patellafemoral ligament，MPFL)作为RPD最常损伤的内侧软组织结构，采用MPFL重建术治疗髌股关节不稳逐渐被接受。Hautamaa等基于生物力学的试验证明：MPFL对髌骨外移起主要限制作用，如术中仅将MPFL切断，髌骨向外侧移位将超过50%。治疗RPD的方法多种多样，一项Fithian等的研究表明治疗RPD的手术方式已超过100种。MPFL重建术从最初的单束重建逐渐发展为与MPFL解剖结构更接近的双束重建。Placella等的解剖学试验证实，MPFL双束重建对恢复MPFL的解剖学形态效果明显优于MPFL单束重建，MPFL单束重建时由于髌骨骨折，可出现高达11%的固定失败率，这可能是由于MPFL单束重建较MPFL双束重建而言应力更为集中所致。Mochizuki等发现半腱肌腱并不能完美替代MPFL，因为其近端纤维束与股中间肌相连，远端纤维连接于髌韧带中部，并非直接连接于髌骨。因此，MPFL术中单、双束重建均无法达到真正意义上的解剖重建，仅为狭义的解剖重建。目前对于MPFL重建术中移植物种类(自体肌腱、同种异体肌腱及人工韧带等)、固定方式的选择(骨道固定法、缝合锚钉固定法、缝合固定法、螺钉固定法及几种方法的结合)、移植物紧张度的调节(是否在关节镜下调节、是否需要适度“矫枉过正”)、固定位置选择等方面仍存在较大争议。现就对MPFL重建术治疗RPD时不同髌骨侧固定方式的优劣、是否应联合外侧支持带松解、关节镜的使用、移植物张力的调节把握以及MPFL重建术治疗RPD的局限性展开综述。
