骺板牵开术治疗儿童肱骨近端近骺板恶性肿瘤

摘要目的分析骺板牵开技术治疗儿童肱骨近端近骺板恶性肿瘤临床疗效。方法自2007年2月至2013年1月，解放军第180医院小儿骨科共收治6例肱骨近端近骺板恶性肿瘤患儿，术前病理确诊普通型骨肉瘤2例，成骨型骨肉瘤1例，成纤维型骨肉瘤1例，毛细血管扩张型骨肉瘤1例，小细胞骨肉瘤1例。所有患儿均在接受新辅助化疗方案后，顺序进行骨骺牵开术、瘤段切除术和骨移植重建术。术后采用美国肌肉骨骼肿瘤协会(musculoskeletal tumor society，MSTS)评分、多伦多保肢评分(toronto extremity salvage score，TESS)、日本骨科协会(Japanese orthopaedic association，JOA)肩关节功能评分和Mayo肘关节功能评分进行疗效评价。结果所有患儿获得随访18～112个月，平均51.0个月，均未出现局部肿瘤复发；骺板牵开时间8～16 d，平均13.0 d；术后MSTS评分为53～97分，平均79分；TESS评分为71～92分，平均83分；JOA肩关节功能评分为66～91分，平均84分；Mayo肘关节评分55～95分，平均91分。结论治疗儿童肱骨近端近骺板恶性肿瘤，骺板牵开术是值得推荐的保肢方法，既可以提供安全的肿瘤切除界限，也可以最大限度地保留骺板的生长能力，临床上有显著优点，但需要严格把握适应证。
