收肌管联合IPACK阻滞对前交叉韧带重建术后镇痛和关节功能的影响

摘要目的〓评估收肌管联合腘动脉与膝关节后囊间隙（infiltration between the popliteal artery and capsule of the knee，IPACK）阻滞对关节镜下前交叉韧带重建术后镇痛和关节功能的影响。方法〓2019年2月至2022年2月东南大学附属徐州市中心医院运动医学科收治前交叉韧带断裂行关节镜下前交叉韧带重建术患者60例，随机分为两组，其中观察组(收肌管+IPACK阻滞)30例，男18例，女12例；年龄18~40岁，平均(24.43±6.39)岁。对照组(收肌管阻滞)30例，男17例，女13例；年龄18~38岁，平均(26.23±5.46)岁。记录两组患者术前、术后6 h、12 h、24 h、48 h静息和动态数字评分法(numeric rating scales，NRS)评分，术后24 h膝关节主动活动范围，术后48 h自控静脉阵痛按压泵(patient-controlled intravenous analgesia，PCIA)按压次数和地佐辛使用人数。结果〓观察组术后6 h、12 h、24 h静息和动态NRS评分均低于对照组，差异有统计学意义(P＜0.05)；但术后48 h两组患者静息和动态NRS评分结果差异均无统计学意义(P＞0.05)。观察组术后6 h、12 h、24 h的PCIA按压次数少于对照组，差异有统计学意义(P＜0.05)；但术后48 h两组按压PCIA次数差异无统计学意义(P＞0.05)。术后24 h观察组患者膝关节活动范围大于对照组，差异有统计学意义(P＜0.05)。观察组术后48 h使用地佐辛占比26.7%，对照组占比53.3%，差异有统计学意义(P＜0.05)。结论〓收肌管联合IPACK阻滞有利于减少前交叉韧带术后疼痛，减少阿片类药物使用，允许术后早期活动，有利于术后加速康复。
