摘要 目的 探讨关节镜下单隧道锁扣带袢钛板套扎固定在前交叉韧带胫骨止点骨折治疗中的临床应用价值。方法 回顾性分析2018年1月至2021年12月上海交通大学医学院附属松江医院行关节镜下骨折复位锁扣带袢钛板套扎内固定治疗的34例前交叉韧带胫骨止点骨折患者资料,其中男22例,女12例;年龄14~60岁,平均(34.2±14.6)岁;受伤至手术时间3~14 d,平均(6.2±2.8) d;根据Meyers-McKeever-Zaricznyj骨折分型:Ⅱ型10例,Ⅲ型18例,Ⅳ型6例。手术前后采用国际膝关节评分委员会(international knee documentation committee,IKDC)评分、膝关节Lysholm评分、膝关节Tegner评分及膝关节活动度(range of motion,ROM)评估膝关节功能,通过膝关节Lachman试验评估膝关节稳定性。记录手术时间、住院时间及术后并发症等情况。结果 34例均获随访,随访时间10~24个月,平均(15.0±4.4)个月;手术时间30.2~43.4 min,平均(35.8±3.2) min。34例患者均顺利手术,1例患者于术后10个月出现膝关节Lachman试验弱阳性。末次随访时膝关节IKDC评分、Lysholm评分、Tegner评分及膝关节ROM分别为(95.56±5.42)分、(96.54±5.34)分、(8.29±2.14)分及(110.28±10.42) °,显著高于术前的(39.47±6.53)分、(45.62±5.56)分、(4.65±3.12)分及(81.25±12.24) °,差异有统计学意义(P<0.01)。结论 关节镜下单隧道锁扣带袢钛板套扎内固定治疗ACL胫骨止点骨折具有疗效确切、操作简易、易拆除等优点,具有较高的临床应用价值。
