摘要 目的 对使用反向Rigidfix横穿钉系统重建前交叉韧带的临床疗效及安全性进行评价分析。方法 回顾性分析2018年1月至2019年1月四川省骨科医院使用反向Rigidfix横穿钉系统重建前交叉韧带患者138例,男91例,女47例;年龄14~56岁,平均(31.44±10.14)岁。通过术前及随访结束时Teger评分、Lysholm评分、KT-1000测值、轴移试验、恢复至伤前运动等级的概率进行比较,评价此技术的临床疗效,通过术后感染、横穿钉断钉或失效、神经血管损伤、膝关节功能障碍、横穿钉穿出的发生率以及横穿钉穿出引起的影响评价其安全性。结果 138例患者中125例完成随访,平均随访(14.54±2.10)个月。随访结束时Teger评分(6.33±0.97)分,Lysholm评分(94.89±4.91)分,KT-1000测量值(1.06±0.71)mm,轴移试验0级118例,Ⅰ级7例,与术前相比差异均有统计学意义(P<0.001),运动能力、生活质量、膝关节稳定性得到明显改善,112例(89.6%)恢复至伤前运动等级。所有患者术后均无感染、横穿钉断钉、横穿钉失效、神经血管损伤发生。完成手术的138例中,28例(20.29%)出现横穿钉穿出,其中3例(2.17%)引起软组织损伤,7例(5.07%)出现软骨损伤;存在横穿钉穿出的患者与其他患者在性别方面比较差异有统计学意义(P<0.001),有横穿钉穿出的主要为女性,其余比较差异无统计学意义(P>0.05)。结论 在使用腘绳肌重建前交叉韧带的手术中,通过前内侧入路反向放置Rigidfix横穿钉系统制备横穿钉隧道,临床疗效确切,医源性损伤明显降低。此技术既可以发挥横穿钉原有的优势,同时与传统Rigidfix技术相比更加安全、有效,值得推广。
