Bikini切口与传统切口直接前方入路髋关节置换术的效果比较

摘要目的〓比较传统切口和Bikini切口微创直接前方入路髋关节置换术的短期临床效果。方法〓纳入2020年1月至2022年6月江苏省滨海县中医院行微创直接前方入路髋关节置换术的45例患者，其中男17例，女28例；年龄40~70岁，身体质量指数(body mass index，BMI)19~28 kg／m2。Bikini切口组22例，传统切口组23例，比较两组术中损肌肉损伤、手术时间、出血量、术后伤口评分、疼痛视觉模拟评分(visual analogue scale，VAS)、引流量以及术后髋关节Harris评分和并发症发生率。结果〓随访时间6~18个月，平均(16±2)个月。Bikini组手术时间较传统组长，Bikini组术后引流量少于传统组，术后第3个月伤口评分优于传统组，差异均有统计学意义(P＜0.05)。Bikini组有5例出现股外侧皮神经损伤，占组内病例数的21.74%，占总病例数的11.11%，明显低于传统组的40.09%、20.00%，差异有统计学意义(P＜0.05)。结论〓直接前方入路髋关节置换中，采用Bikini切口与传统切口有相似的手术效果，但前者术后伤口愈合更佳，股外侧皮神经损伤更少。
