氨甲环酸和止血带在全镜下前交叉韧带双束重建术中的应用

摘要目的探讨氨甲环酸和止血带在关节镜下前交叉韧带(anterior cruciate ligament，ACL)重建术围术期间应用的效果及风险分析。方法选取2020年3月至2021年1月北部战区总医院诊治的军事训练伤致ACL损伤病例49例，其中男47例，女2例；年龄19~43岁，平均(29.0±7.0)岁。均行ACL双束重建手术。采用随机数字表法进行分组：氨甲环酸组(术中使用氨甲环酸)16例，止血带组16例(术中使用止血带)16例，联合组(术中同时使用氨甲环酸和止血带)17例。比较三组手术时间、疼痛视觉模拟评分(visual analogue scale，VAS)、患肢肿胀程度、术后3 d血炎性指标(白介素6、肿瘤坏死因子α、氧自由基)和术后不良反应发生情况。结果术后3 d氨甲环酸组VAS、患肢肿胀程度、血炎性指标显著低于其他组，差异有统计学意义(P＜0.05)；三组手术时间相比差异无统计学意义(P＞0.05)。结论关节镜下ACL重建术中单独使用氨甲环酸足够满足手术操作需求，止血带的使用会增加术后患肢疼痛和肿胀。推荐术中应用氨甲环酸时视情况应用止血带，应尽量减少止血带使用时间。
