关节腔内注射玻璃酸钠对胫骨平台骨折术后膝关节炎症及功能影响

摘要目的探讨膝关节腔内注射玻璃酸钠对胫骨平台骨折术后关节内早期炎症及关节功能恢复的影响。方法 2016年7月至2018年2月期间在雷州骨科医院治疗的胫骨平台骨折患者68例，男42例，女26例，随机分为对照组及玻璃酸钠组，各组手术当天及术后第1周收集关节腔积液，酶联免疫吸附测定(enzyme linked immunosorbent assay，ELISA)检测白细胞介素-1β(interleukin-1β，IL-1β)、白细胞介素-6(interleukin-6，IL-6)、肿瘤坏死因子-α(tumor necrosis factor，TNF-α)表达情况，术后第1周采用视觉模拟评分(visual analogue scale，VAS)评估患膝疼痛，测量关节活动度(range of motion，ROM)，术后3个月膝关节功能采用美国特种外科医院(hospital for special surgery，HSS)评分。结果与对照组比较，玻璃酸钠组术后第1周患膝关节静息疼痛VAS评分较低(P＝0.030)，膝关节ROM较大(P＝0.009)。术前两组关节腔积液IL-1β、IL-6、TNF-α表达比较，差异无统计学意义(P＞0.05)，但是术后第1周玻璃酸钠组关节腔积液IL-1β、IL-6、TNF-α表达较对照组降低(P＝0.017、0.011及0.041)。术后3个月随访见玻璃酸钠组患膝关节HSS评分较对照组高(P＝0.031)。结论胫骨平台骨折术后玻璃酸钠关节腔内注射可以抑制早期关节内炎症，缓解疼痛，促进患膝功能恢复。
