Abstract:Objective To analyze the clinical efficacy of modified posterior approach in the treatment of cervical spondylotic myolopathy with radiculopathysis and biomechanical parameters and provide a reference for the selection of clinical treatment options for spinal cord and nerve root type cervical spondylosis.Methods One hundred patients with cervical spondylotic myolopathy with radiculopathysis admitted in our hospital from January 2015 to January 2016 were selected as the study subjects.All patients were divided into control group and observation group according to principle of random distribution,with 50 cases in each group. Patients in control group were treated with anterior posterior surgery,while observation group were treated with modified posterior surgery.All patients were followed up for more than 2 years.The Japanese Orthopaedic Association spinal function score (JOA score),JOA score improvement,and cervical functional disability index table (NDI) before operation and 6 months,1 year,and 2 years after treatment were compared among the 3 groups of patients.Scores,visual analog scale scores (VAS),ROM (cervical mobility),and PROM (passive motion of the cervical spine).The efficacy of radical symptom was evaluated and compared 6 months after operation.Results There was no significant difference in the JOA score,cervical ROM and cervical muscle tension between two groups (P>0.05).The JOA score,JOA score improvement rate,ROM,and muscle tension of observation group at 6 months,1 year,and 2 years after operation were significantly higher than those in control group(P<0.05),and the NDI and VAS scores were significantly lower than those in control group (P<0.05).After 6 months of operation,the excellent and good rate of root symptoms in observation group was significantly higher than that in control group (P<0.05).Conclusion The improved posterior surgery has a definite curative effect and high safety in the treatment of cervical spondylotic myelopathy and cervical spondylosis.It resolves the spinal cord and nerve root compression in one procedure,relieves the patient's pain and financial burden.It is of great clinical value for patients with cervical spinal cord and nerve root type cervical spondylosis.
