Abstract Objective To explore the efficacy of Ilizarov technique in the extension of Brachydactyly D type disease. Methods From September 2018 to August 2020,4 patients with Brachydactyly D type disease were lengthened by unilateral external fixator designed according to Ilizarov technology.Among them, there were1 male caseand 3 female cases. The patients aged 22~42 years, with an average of(30.25±0.55)years.There were right thumb short finger in 3 cases, and left in1 case. The length of the affected finger was 5.3~10.2 mm shorter than that of the healthy finger, with an average of (8.25±0.22)mm shorter.Using the lizarov technique combined with the phalangeal base osteotomy scheme, the threaded rod on the Ilizarov was rotated 7 days after operation, and the phalanx was gradually drawn and extended. The motion of the hand joint (total active movement,TAM) was evaluated according to the trial criteria upper limb function evaluation of the Chinese Medical Association, and the phalanx extension length, bone extension ratio and complications were observed. Results All 4 patients were followed up for 6~13 months, with an average of (7.8±1.5)months.Phalangeal deformities were corrected in all patients, lengthening callus complete calcification, phalanx extension length 5.5~10.2 mm,average(8.25±0.22)mm.According to the TAM system evaluation method, the results were all excellent and no complications occurred. Conclusion The unilateral miniature external fixation frame designed according to the principle of Ilizarov technology extends the phalanx and follows the concept of natural bone reconstruction. It is an effective method for the treatment of Brachydactyly D type disease and is worthy of clinical promotion.
