牵引床辅助下微创治疗Schatzker Ⅴ、Ⅵ型胫骨平台骨折

Abstract Objective To investigate the clinical effect of traction assisted minimal invasive treatment of Schatzker Type V，VI tibial plateau fractures.Methods 32 patients with Schatzker type Ⅴ，Ⅵ fractures were divided into two groups：the minimally invasive treatment group ( observation group) and the traditional open reduction and internal fixation ( control group)，with 16 cases in each group.The observation group was treated with traction assisted reduction and minimally invasive plate fixation.We collected intraoperative situation (including operation time，intraoperative blood loss and incision length)，the postoperative situation (postoperative complications such as skin infections，Knee stiffness，and traumatic arthritis)，and the last follow-up for osteo-arthritis index (WOMAC) score and the United States，New York special surgical hospital data (HSS) score.After summarizing the statistical results，the difference of efficacy between the two groups was compared.Results All patients were followed up for 4 to 6 months.The observation group of operation time，intraoperative blood loss，incision length were significantly less than the control group，the differences were statistically significant(P＜0.05).The incidence of postoperative complications in the control group was 33.33％，while the incidence of postoperative complications in the observation group was only 6.67％，and the difference between the two groups was statistically significant (P＜0.05).The osteoarthritis index (WOMAC) score and the New York special surgical hospital (HSS) score were analyzed，observation group were superior to control group，the differences between the two groups have statistical significance (P＜0.05).Conclusion Compared with traditional open reduction，the minimally invasive treatment of Schatzker V and VI tibial plateau fractures with traction greatly reduced for soft tissue injury，shorten the operation time，and the incidence of postoperative complications is lower，and the joint function can recover well，which is worthy of clinical promotion.
