脊柱手术围手术期隐性失血原因分析

摘要术中出血是外科手术中常见的问题，尤其在脊柱融合术中，失血过多可能需要输血，在成年人脊柱融合手术中输血率达50％～81％不等。当我们提到脊柱围手术期的失血量时，通常是指显性失血即术中出血量和术后引流量，然而从临床观察发现存在与围手术期间显性失血量不符合的贫血症状，这可能与隐性失血量有关。脊柱围手术期隐性失血量比例大，是造成术后血红蛋白不同程度下降的主要原因之一。脊柱手术中主要有年龄、手术时机、疾病种类及手术方式、麻醉学等多因素影响，同时体位方面脊柱手术主要采用俯卧位增加腹腔压力，腰椎Batson静脉丛压力升高，增加隐性失血量。因此，脊柱围手术期需重视隐性失血量及其影响因素，术前充分评估、术中仔细止血，合理使用止血药物，有效减少围手术期隐性失血量，提高患者安全性，降低术后并发症及输血带来的相关传染性疾病等。目前最新研究表明隐性失血与中性粒细胞产生的活性氧有关，故在围手术期间使用氨甲环酸等止血药的同时结合抗氧化剂药物可以减少隐性失血量。
