Sanders Ⅲ型跟骨骨折治疗中外侧腓肠神经下弧形切口的应用

摘要目的〓探究跟骨外侧腓肠神经下弧形切口在治疗Sanders Ⅲ型跟骨骨折的临床应用分析。方法〓回顾性分析2018年10月至2022年4月黄山市人民医院创伤骨科收治的49例Sanders Ⅲ型跟骨骨折患者，均采用解剖钢板内固定治疗，根据手术切口不同分为观察组和对照组。观察组25例，采用跟骨外侧腓肠神经下弧形切口，男24例，女1例；年龄23～73岁，平均(50.24±10.50)岁。对照组24例，采用跗骨窦切口，男22例，女2例；年龄32～75岁，平均(51.62±9.99)岁。对比两组手术时间、术后跟骨高度、跟骨Blher角、跟骨Gissane角、术后并发症及美国矫形外科足踝协会Maryland足部功能评分。结果〓患者均获得随访，随访时间8~18个月，平均(12.08±1.83)个月，骨折均愈合。观察组手术时间(73.08±18.25)min短于对照组(86.13±22.81)min，差异有统计学意义(P＜0.05)。两组术后跟骨Blher角、跟骨Gissane角比较差异无统计学意义(P＞0.05)，两组术后跟骨高度比较差异有统计学意义(P＜0.05)。两组Maryland足部功能评分优良率比较，观察组(88.46%)高于对照组(70.83%)，其中跟骨外观、穿鞋、上楼梯及地面要求差异有统计学意义(P＜0.05)。结论〓跟骨外侧腓肠神经下弧形切口手术操作简单、手术时间短，能直视下复位跟骨外侧体部骨折，更好地纠正内翻畸形，恢复跟骨高度及后足力线，术后并发症低，更有利于足踝功能恢复，可作为治疗Sanders Ⅲ型跟骨骨折手术切口的一种选择。
