韧带修复联合微骨折术治疗慢性踝关节外侧不稳伴距骨骨软骨损伤

摘要目的探讨全镜下运用套索环缝合技术行距腓前韧带修复联合微骨折术治疗慢性踝关节外侧不稳伴小面积距骨骨软骨损伤的早期临床疗效。方法回顾性分析广州中医药大学附属中山中医院2019年4月至2022年6月在全镜下行距腓前韧带修复联合微骨折术治疗的慢性踝关节外侧不稳伴距骨骨软骨损伤患者30例，其中男21例，女9例；年龄18～59岁，平均(37.0±12.0)岁。对比术前及末次随访患者疼痛视觉模拟评分(visual analogue scale，VAS)、美国足踝外科协会(American orthopaedic foot and ankle society，AOFAS)评分、Karlsson-Peterson评分、距骨骨软骨损伤Hepple分期、距骨前移距离及其倾斜角来评价整体手术疗效。结果所有患者均顺利完成手术，切口全部一期愈合。术后随访时间8～18个月，平均(12.5±2.8)个月。术后及随访过程中未观察到明显并发症，踝关节外侧肿痛等症状消失，无反复扭伤表现，内翻应力试验、前抽屉试验均为阴性，日常工作和运动能力均得以恢复。末次随访时患者VAS评分、AOFAS评分、Karlsson-Peterson评分、距骨骨软骨损伤Hepple分期、距骨前移距离及其倾斜角均较术前改善，差异有统计学意义(P＜0.001)。结论全镜下运用套索环缝合技术行距腓前韧带修复联合微骨折术治疗慢性踝关节外侧不稳伴小面积距骨骨软骨损伤的近期临床疗效满意，是一种安全可行的手术治疗方法。
