摘要 腰椎退变性疾病是指腰椎及其附属结构随年龄增长自然老化或长期劳损加速退变导致的一系列病理性改变。随着人们生活方式和工作环境的转变,腰椎退变的发病率呈现出逐渐增加的趋势。近年来研究表明,人口老龄化、长时间保持坐姿而缺少运动、长期从事重体力劳动均与该病的发生密切相关。
腰椎退变性疾病可引起腰背痛伴双下肢麻木放射痛,严重者出现大小便功能障碍、鞍区麻木、性功能障碍等马尾神经损伤症状,影响工作能力与生活质量。对于病程长、症状反复影响工作和生活、神经症状进行性加重或合并马尾神经损伤的腰椎退变性疾病患者,通常采取手术治疗。传统的开放手术作为腰椎退变性疾病的经典治疗手段,尽管能将病灶充分暴露,达到彻底减压的目的,但手术出血多、对正常组织破坏大、术后并发症多等缺陷也不可忽视。因此,微创脊柱外科技术应运而生,该技术不仅患者疗效满意,而且具有术中对正常解剖结构破坏更少、手术切口更小、术后恢复更快、住院时间更短、大幅降低医源性并发症的发生率等优点。基于这些优点,微创理念已逐渐深入人心。
单侧双通道内镜技术(unilateral biportal endoscopy,UBE)作为当下脊柱微创领域研究的热点,近年来在全球脊柱外科医师的不断研究和应用中飞速发展,在腰椎髓核摘除、椎管减压治疗方面疗效显著,在腰椎融合术方面也逐渐拓展,为腰椎退变性疾病的治疗做出极大地贡献。
