先天性肌性斜颈患儿运动发育异常的临床研究进展

摘要先天性肌性斜颈(congenital muscular torticollis，CMT)是一种常见的先天性疾病，主要以一侧胸锁乳突肌挛缩导致的头向患侧偏斜、颜面向健侧旋转的头颈部异常姿势为特点，通常会引起颈椎活动受限，颜面、头颅、枕部不对称等，可能存在髋关节发育不良、继发代偿性脊柱侧凸，甚至因早期治疗不及时导致永久畸形等。CMT的发病率为0.3%~2.0%。目前，对于CMT的病因尚未完全明确，主流学说认为与发育不良、宫内姿势异常、产伤、遗传、细胞因子表达等综合因素有关。临床上对CMT的干预治疗和远期随访通常以关注局部的结构功能发育为主，但近年来随着国内外学者研究的不断深入以及家庭和社会对儿童生长发育的愈发重视，CMT患儿在局部畸形治愈后的远期运动发育状况逐渐成为临床医生及患儿家长共同关心的问题之一。因此，正确认识CMT与运动发育之间的关系，对完善CMT患儿的治疗措施具有重要的意义。本文将近几年来有关CMT患儿与其运动发育特征的研究进行综述整理，期望能够为进一步完善CMT患儿的治疗方案提供新的思路与证据支持。
