摘要目的 探讨应用胫骨骨膜牵张术治疗糖尿病足的临床疗效。方法 选取2020年1月2021年1月中南大学湘雅医学院附属株洲医院收治的19例Wagner分级为2~4级的糖尿病足患者，其中男13例，女6例；年龄50~80岁，平均(58.84±5.80)岁；糖尿病病程(9.63±2.31)年，糖尿病足溃疡病程(9.47±3.29)周。采取胫骨骨膜牵张术进行保肢治疗，记录手术时间、术中出血量等围手术期指标，比较手术前后趾端血氧饱和度、足温、踝肱指数、Wagner分级，采用双下肢CT血管造影术(computed tomography angiography，CTA)评估患肢血运情况、疼痛视觉模拟评分(visual analogue scale，VAS)评估静息疼痛改善情况及健康调查简表(the MOS item short from health survey，SF-36)评价患者术后生活质量改善情况。结果 19例患者均随访3个月。手术时间(41.32±1.9)min，术中失血量(50.21±6.69)mL。术后3个月患者的趾端血氧饱和度、足温、踝肱指数、Wagner分级、VAS及SF-36量表评分较术前均明显改善，差异有统计学意义(P＜0.05)；下肢CTA均可见下肢远端小血管和牵张区域侧枝循环的形成，部分血管向远端延伸。结论 胫骨骨膜牵张术能有效改善下肢血运，促进创面愈合，避免截肢，提高糖尿病足患者的生活质量。
