Abstract:Objective To retrospectively analyze the effects of lumbar rigidity on postural pelvic motion in patients undergoing total hip arthroplasty (THA).Methods Preoperative clinical data of 45 patients who underwent THA in our hospital due to femur head necrosis from October 2018 to October 2021 were retrospectively analyzed.There were 20 elderly males and 25 elderly females,the age ranged from 55 to 78 years old,with an average age of(65.7±6.3)years old.The difference between preoperative pelvic incidence (PI) and lumbar lordosis (LL) PI-LL (≤10° or >10°) patients were divided into two groups.The lumbar rigidity group (21 cases) was defined as PI-LL>10°.In the non-lumbar rigidity group (24 cases),PI-LL was defined as ≤10°.Pelvic tilt (PT),pelvic range of motion (ΔPT),PI and lumbar lordosis (LL) were measured using lateral radiographs of patients in supine,standing and sitting positions.The differences of PT and ΔPT between the two groups were compared.Results The PT of lumbar rigidity group in supine position and standing position was significantly higher than that of non-lumbar rigidity group,the difference was statistically significant (P<0.05),but there was no statistically significant difference between the two groups in sitting position (P=0.44).In the lumbar rigidity group,there was a significant change in pelvic tilt from standing to sitting (-7.17± 12.11)°,whereas in the non-lumbar rigidity group there was a small change in pelvic tilt (-1.04±12.52) °.The difference between the two groups was statistically significant.When patients moved from standing to supine,the ΔPT of lumbar rigidity group (-6.36±4.07) ° was also significantly greater than that of nonlumbar rigidity group (-2.79±5.47) ° (P<0.05).Conclusion Compared with non-lumbar rigidity,patients with lumbar rigidity have significantly higher pelvic motion during posture change.This implies greater variability in acetabular position during dynamic activity and an increased risk of prosthetic dislocation after THA in this patient population.
