摘要目的 分析人工全髋关节置换(total hip arthroplasty，THA)对老年股骨转子间骨折内固定失败的临床疗效。方法 回顾性分析2014年7月至2018年7月我院骨一科收治的21例股骨转子间骨折内固定失败的老年患者资料，所有患者均接受THA。内固定失败类型：5例股骨近端抗旋髓内钉，8例股骨近端锁定钢板，8例动力髋螺钉。其中17例患者获得随访，男9例，女8例；年龄65~76岁，平均(73.8±2.5)岁。记录患者关节置换前后髋关节Harris评分，采用健康调查问卷(the MOS item short from health survey，SF-36)评估患者术后生活质量，随访期间通过髋关节DR片观察假体稳定性。结果 21例中1例因肺栓塞死亡，1例因心衰死亡，1例因术后脑卒中退出随访，1例因个人因素失访。17例患者随访时间19~54个月，平均(26.2±7.1)个月。所有患者术后均未出现关节感染，假体周围骨折或脱位等。患侧髋关节Harris评分从术前的(33.87±11.05)分提高到治疗后24个月的(91.08±5.9)分。SF-36评分术后24个月较术前显著提高(P＜0.01)。结论 对于老年股骨转子间骨折内固定失败的患者，THA可作为一种有效的挽救性治疗方法，可减轻患者髋关节疼痛，恢复髋关节功能，改善患者生活质量。
