骨肉瘤的保肢治疗策略

摘要骨肉瘤(Osteosarcomas，OS)是最常见的原发性骨恶性肿瘤，好发于下肢干骺端。现在OS患者治疗方式为新辅助化疗结合保肢手术，手术方式以广泛性切除为主(见图1)。新辅助化疗提高了患者的生存率，保肢手术拯救了肢体并且保留了功能。保肢手术要求肿瘤“闭合切除”，即原则上在肿瘤所有方向上都有正常组织覆盖，关键在于选择合适的手术边界。复发率则取决于肿瘤对化疗药的反映、手术边界的选择、是否栓塞肿瘤侵犯周围血管。近年来，随着保肢治疗的患者生存率明显提高，长期生存的并发症也越来越多，有报道高达80%，因此需要新的方式减少并发症，提高患者的生存质量。对于复发、转移、耐药的骨肉瘤患者，二线药物及靶向治疗的出现，使其成为可能。
