颈性眩晕发病机制的研究进展

摘要眩晕作为临床常见症状之一，往往涉及全身多个系统，病因复杂，患者主观症状强烈，缺乏有效的诊疗手段。传统观点认为颈性眩晕是以颈部交感神经或椎动脉受刺激所引起。1926年Barre等首先报道了颈部交感神经受颈椎关节等外部刺激可引起头晕、头颈痛等症状。1955年Ryan和Cope根据交感神经和眩晕之间的关系首先提出颈性眩晕这个概念。相关数据显示，颈椎病发病率为3.8%～17.6%，超过6成的老年性眩晕可归因于颈椎病。临床上存在数量庞大的颈性眩晕患者群体，严重影响其日常生活及工作能力。但本病以患者主诉为主要依据，涉及神经内科、耳鼻喉科、心血管内科等多个相关科室，缺乏特异性的辅助检查及临床诊断方法，对于病因尚有争议。笔者总结国内外学者最新研究，将颈性眩晕归类为椎动脉狭窄学说、交感神经刺激学说、血管活性因子学说及颈部本体感受器学说，综述如下。
