婴幼儿与儿童化脓性髋关节炎术后髋部后遗畸形的比较分析

摘要目的比较分析婴幼儿与儿童化脓性髋关节炎关节切开引流术后髋部后遗畸形的临床特点。方法回顾性分析2016年6月至2022年9月济南市儿童医院收治的明确诊断为化脓性髋关节炎患儿的临床资料，共有31例31髋，其中男18例，女13例；年龄9 d~10岁，中位年龄8.5(1.1，65.3)个月；延误治疗2~20 d，中位数为6.0(5.0，10.0)d。所有患儿均采用髋关节切开引流术，术后达到临床愈合标准后出院，之后定期门诊随访。术后随访通过Bennett临床功能分级评估术后髋关节功能。通过末次随访的髋关节正位X线片测量髋臼指数，判断是否有残余髋臼发育不良，Reimer’s外移指数判断是否半脱位，根据Salter标准确定有无股骨头坏死。结果 31例患儿均获随访，随访时间6~36个月，中位随访时间12.0(8.0，12.2)个月。患儿均获影像检查，末次随访影像学评估，发现残余髋臼发育不良15例，半脱位13例，股骨头坏死26例。29例进行Bennett临床功能评分，优3例，良14例，可10例，差2例；术后随访婴幼儿的影像学及临床功能满意率均差于儿童组，差异有统计学意义，其中婴幼儿组发生残余髋臼发育不良及半脱位的概率显著大于儿童组，而股骨头坏死的发生率两者大致相当。结论化脓性髋关节炎术后有较高的髋部后遗畸形发生率，婴幼儿与儿童相比更易继发髋臼发育不良、半脱位、股骨头坏死等后遗畸形，髋关节功能也较差。
