Ottobock外展支具早期治疗儿童发育性髋关节发育不良疗效观察

摘要目的观察Ottobock外展支具早期治疗＜6个月儿童发育性髋关节发育不良的临床疗效。方法自2015年12月至2020年3月采用Ottobock外展支具早期治疗＜6个月发育性髋关节发育不良患儿40例，其中男6例，女34例；左侧18例，右侧10例，双侧12例；年龄1~6个月。所有患儿行髋关节超声检查，诊断为Graf Ⅱb型以上。佩戴Ottobock支具治疗。每2周复查髋关节超声，＜6个月超声结果为GrafⅠ型后停止佩戴，＞6个月复查骨盆正位X线片髋臼指数小于25°时停止佩戴。结果 36例患儿获得随访，随访时间15~48个月，平均(31.8±11.7)个月。支具佩戴时间1~4个月。34例共45髋获得了改善，复查髋关节超声为Graf Ⅰ型。2例Graf Ⅳ型儿童复位失败，佩戴支具1个月后改为闭合复位人类位石膏外固定治疗。随访结束时髋臼残余发育不良3例4髋(双侧1例2髋，左侧2例2髋)。2例2髋(双侧1例右髋，左侧1例1髋)出现股骨头缺血性坏死。随访结束时髋关节改善优良率为91.5%。结论 Ottobock支具早期治疗儿童发育性髋关节发育不良安全有效。对于Graf Ⅳ型儿童复位率较低，闭合复位人类位石膏外固定治疗是支具治疗失败后的一种更好的选择。
