手法松解联合关节镜手术及康复训练治疗原发性冻结肩的近期疗效分析

摘要目的探讨手法松解联合关节镜手术及康复训练治疗原发性冻结肩的临床疗效。方法选取我科2016年6月至2019年6月收治的原发性冻结肩患者48例，其中男性21例，女性27例，年龄42～65岁，平均(50.1±5.4)岁；所有患者术前均保守治疗3个月以上，术中全麻下行手法松解联合关节镜手术；术后第1天开始进行康复功能训练；所有患者随访6个月以上，记录术前及末次随访肩关节活动度(range of motion，ROM)、ConstantMurley肩关节功能评分(ConstantMurley score，CMS)及疼痛视觉模拟评分(visual analogue scale，VAS)，分析近期疗效。结果 48例患者术中及术后无明显并发症，均获得6个月以上随访；末次随访时所有患者的肩关节ROM、CMS及VAS评分均明显改善。患肩外展上举角度由(52.8±6.4)°改善至(153.6±8.0)°，前屈上举角度由(73.2±6.3)°改善至(153.9±10.4)°，体侧位外旋角度由(5.5±1.2)°改善至(58.2±5.0)°，内旋角度由(35.5±5.4)°改善至(61.2±6.2)°；CMS评分由(22.6±3.5)分改善至(76.1±8.2)分；VAS评分由(5.7±0.9)分改善至(2.4±1.1)分；所有 P 值均小于0.001，差异有统计学意义。结论手法松解联合关节镜手术及康复训练治疗原发性冻结肩安全、创伤小、恢复快，能显著改善肩关节活动度，有效缓解疼痛，缩短病程，取得良好的近期疗效。
