Abstract Objective To observe the clinical effect of the Ilizarov bone transport technique combined with antibiotic calcium sulfate after diseased bone resection in the treatment of chronic osteomyelitis.Methods A retrospective analysis of 17 patients with chronic osteomyelitis were treated using Ilizarov bone transport technique combined with antibiotic calcium sulfate from January 2018 to August 2019.All patients underwent diseased bone resection of osteomyelitis,circular or unilateral external fixation,antibiotic calcium sulfate implantation in the bone defect,and Ilizarov bone transport technique to repair the bone defect.Results All patients were followed up for 10 to 15 months(average:12.7±1.6 months).Among the 17 patients,diseased bone resection of femur in 3 cases and tibia in 14 cases.A circular external fixator was installed in 7 cases and unilateral external fixator was installed in 10 cases.The femurswerefixed with unilateral external fixator,and the tibiaswere fixed with circular external fixator or unilateral external fixator.If the bone defect was large after resection of the diseased bone segment,the circular external fixator would be selected when the of unilateral external fixator couldnotprovide enough stability or can not be installed.All patients were implanted with antibiotics calcium sulfate during the operation.There were 8 cases of primary bone transport and 9 cases of secondary bone transport.The time of bone transport was 2~5 months(average:3.5±1.4 months),the mineralization time after bone transport was 6~14 months(average:8.3±1.8 months),the time of external fixation was 9~15 months(average:12.3±2.7 months),and the external fixation index was 1.2~2.6 month/cm(average:1.5±0.3 month/cm).The absorption time of calcium sulfate was 1~2 months (average:1.6±0.2 months).There were 5 cases of nail infection and 1 case ofhorseshoe foot deformity after the operation.There was no vascular and nerve injury in all patients.The effective rate of treatment was 100%.According to the Paley standard,the excellent and good rate of functional recovery of the affected limb was 94.12%.There was no significant difference in the range of motion of knee joint,but the quality of life was significantly improved after surgery.Conclusion Ilizarov bone transport technique combined with antibiotic calcium sulfate can not only perform radical surgery for osteomyelitis,but also improve the clinical efficacy and quality of life of patients.
