Abstract Objective To investigate the efficacy of arthroscopic treatmentby posterior approach in the treatment of popliteal cysts and compare it with traditional open excision. Methods A retrospective study of 68 patients with popliteal cysts (30 males and 38 females) admitted to our hospital from January 2014 to January 2019,The patients aged 33~61,with an average of (43.01±5.39).Among them,32 cases underwent arthroscopic surgery and 36 cases underwent open excision. The operation time, length of the incision, duration of hospital stays, and recurrence were compared between the two groups. The patients were treated with Rauschning and Lindgren’s axillary cyst grading scale,Lysholm knee score, and pain visual analogue scale (VAS).Results The mean duration of follow-up was (8.46±2.40)months (range 5~12 months). The joint symptoms were relieved and the joint function was improved. There was no cyst recurrence in the arthroscopic surgery group. Two patients in the open excision group relapsed from 3 to 6 months after surgery, and then under arthroscopy. The symptoms disappear. The length of the incision, the hospitalization time, and recurrence rate, knee joint VAS,Lysholm knee joint score,Rauschning and Lindgren’s cyst grading scores were significantly better in the arthroscopy group than in the open excision group, and the difference was statistically significant (P<0.05).Conclusion Arthroscopic posterior approach for the treatment of popliteal cyst is a safe and effective treatment method.
