摘要目的 本研究旨在评估关节镜下关节囊松解术对确诊为“冻结期”原发性冻结肩患者的短期疗效。方法 选择2016年4月至2018年1月收治入院并行关节镜下关节囊松解术的20例原发性冻结肩患者,其中男性12例,女性8例,年龄46~74岁,平均(59.5±7.41)岁。所有患者经术前临床诊断为“冻结期”原发性冻结肩,病程7~13个月,平均(9.58±1.68)个月。术后疗效评估方法包括疼痛视觉模拟评分(visual analog scal,VAS)、Constant-Murley肩关节功能评分(Constant-Murley score,CMS)、肩关节活动度(range of motion,ROM)。结果 术后所有患者均得到随访,随访时间为术后1个月、2个月、6个月。术后VAS评分明显降低,由术前的(5.10±0.72)分降低到术后6个月的(1.35±0.75)分(P<0.01)。术后CMS评分显著提高,由术前的(33.95±4.59)分提高到术后6个月的(85.75±6.15)分(P<0.01)。结论 关节镜下关节囊松解术对"冻结期"冻结肩患者治疗安全、有效,短期疼痛缓解、关节功能评分及活动度明显提高。
Abstract:Objective To assess the short-term effectiveness of arthroscopic capsular release in a cohort of patients with a precise diagnosis of idiopathic frozen shoulder at frozen stage.Methods Twenty patients (12 male,8 female) with frozen shoulder who were treated with joint capsule release under arthroscopy from Apr.2016 to Jan.2018 were prospectively enrolled in this study.All patients were clinically diagnosed as idiopathic frozen shoulder of frozen stage The patients’ ages ranged from 46 to 74 years(59.5±7.41 years),and the duration from the onset of shoulder pain to the examination was 7 to 13 months (9.58±1.68 months).Outcome measures which including the Visual Analogue Scale (VAS),range of motion (ROM),and Constant-Murley Score (CMS) were evaluated pre-operative and at 1,2 and 6 months after operation.Results All patients were followed after operation,statistically significant improvement were found in VAS from (5.10±0.72) to (1.35±0.75) 6 months after operation,and CMS improved from (33.95±4.59) to(85.75±6.15) 6 months after operation.Conclusion Arthroscopy release for the frozen shoulders at frozen stage is a safe and effective procedure with rapid improvement in pain,function and range of motion.
