双膝关节一期置换术中静脉及局部应用氨甲环酸的对照研究

摘要目的探讨双膝关节一期置换术中氨甲环酸给药方式对患者的影响。方法选取2014年8月至2016年8月于临汾市第四人民医院骨科行一期双侧膝关节置换术的患者。根据纳入排除标准共纳入75例，通过使用氨甲环酸(tranexamic acid，TEA)方式不同分为三组，每组25例。A组术中在第一侧全膝关节置换(total knee arthroplasty，TKA)放止血带前10 min静脉点滴1 g氨甲环酸，术后3h视情况决定是否再次应用；B组双膝TKA术中均在缝合关节囊后经引流管注射氨甲环酸0.5 g/50 mL并夹闭引流管2 h；C组双膝术中在第一侧TKA放止血带前10 min静脉点滴1 g氨甲环酸，同时缝合关节囊后经引流管注射氨甲环酸0.5 g/50 mL并夹闭引流管2 h。比较三组患者引流量，术前、术后血红蛋白(hemoglobin，Hb)、红细胞压积(hematocrit，Hct)、隐性失血量、术后输血率及下肢深静脉血栓发生率。结果三组患者中B组在第1天引流量最多(644.1±97.8)mL，C组(418.7±92.4)mL最少，但第2天引流量B组最少(148.6±99.7)mL，C组最多(167.5±87.5)mL，B组较其他两组总引流量(792.7±94.6)mL最多。三组患者引流量、第1天引流量比较，差异有统计学意义(P＜0.05)；第2天引流量比较，差异无统计学意义(P＞0.05)。输血率中B组最多(80％)，C组最少(48％)，B组较A、C组差异有统计学意义(P＜0.05)。且三组均无患者发生双下肢深静脉血栓。结论氨甲环酸局部联合静脉应用于双膝关节一期置换术，可以有效减少引流量，降低输血率，并不增加血栓发生率。
