全膝关节置换术治疗伴关节外畸形的重度内翻膝骨关节炎

摘要目的〓探索初次人工全膝关节置换术(total knee arthroplasty，TKA)中应用髁限制性膝假体和关节内代偿截骨治疗伴有关节外畸形的重度内翻膝骨关节炎中，同时恢复下肢机械力线和关节平衡的疗效及安全性。方法〓回顾性分析2020年12月至2022年3月赤峰市医院行TKA治疗的24例(27膝)重度内翻膝骨关节炎患者资料，其中右膝13例，左膝8例，双膝3例；男5例，女19例；年龄53~74岁，平均(64.00±5.43))岁；股骨远端生理外翻(hip-knee shaft，HKS)角为8 °~13 °，平均(9.70±1.44)°。术前和末次随访时，比较股骨远端机械外侧角(mechanical lateral distal femur angle，mLDFA)、髋-膝-踝(hip-knee-ankle，HKA)角、机械轴偏向(mechanical axis deviation，MAD)评估下肢机械力线恢复情况，比较美国膝关节协会(knee society score，KSS)评分、疼痛视觉模拟评分(visual analogue scale，VAS)、膝关节活动度(range of motion，ROM)评估疗效，行膝内、外翻应力试验评估内侧副韧带张力和关节稳定情况。结果〓24例患者均获随访，随访时间12~28个月，平均(19.41±4.77)个月。膝内、外翻应力试验均阴性，均未发生内侧副韧带松弛、断裂、关节不稳、假体松动、假体周围骨折、感染等并发症，3例出现小腿肌肉内静脉血栓。末次随访时mLDFA、HKA角、MAD及膝关节ROM、VAS评分、KSS膝关节评分和KSS膝功能评分均较术前显著改善，差异有统计学意义(P＜0.05)。结论〓TKA治疗伴有关节外畸形的重度内翻膝骨关节炎中应用应用髁限制性膝假体和关节内代偿截骨可一次手术同时恢复下肢机械力线和关节平衡，疗效可靠且安全。
