机械性吸-呼气技术在脊髓损伤呼吸管理中的应用

摘要目的将国内外研究关于机械性吸-呼气技术(mechanical insufflation-exsufflation，MIE)应用于脊髓损伤(spinal cord injury，SCI)患者中的文献进行系统性分析，为脊髓损伤呼吸管理和机械性吸-呼气技术的临床应用提供依据。方法计算机检索Cochrane Library、Pubmed、Embase、中国知网、万方数据库收录的期刊和学术论文，对机械性吸-呼气技术在脊髓损伤以及其他应用的文献进行分析。结果获得机械性吸-呼气技术在脊髓损伤中的应用的临床研究14篇，其中随机对照和随机交叉试验2篇，前后比较和回顾性分析研究9篇，个案报道3篇。机械性吸-呼气技术的其他应用研究102篇。所得文献数量太少不足以进行Meta分析。结论 MIE在提高有效排痰率、减少有创性介入，提高慢性SCI患者舒适度和生活质量、减少肺部感染、重复住院次数和住院花费方面证据尚不足，但有较大的潜力和研究空间。
