全膝关节置换术中灌注“鸡尾酒”疗法的优势分析

摘要目的分析探讨灌注“鸡尾酒”注射疗法对全膝关节置换术的临床疗效。方法回顾性选取清华大学附属北京清华长庚医院2018年1月至2019年12月行全膝关节置换患者102例，按照“鸡尾酒”注射途径的不同分为灌注治疗组及浸润治疗组。灌注治疗组52例，男11例，女41例；平均年龄(68.2±6.4)岁。浸润治疗组50例，男16例，女34例；平均年龄(63.8±12.5)岁。分析两组间患者术后血红蛋白、血白蛋白及膝关节疼痛评分的差异。结果两组患者术后血红蛋白及血白蛋白差异无统计学意义，但是术后24h灌注治疗组的VAS评分明显低于浸润治疗组。结论虽然灌注“鸡尾酒”注射治疗在术后控制隐性出血等方面无法较传统方法产生明显优势，但是其可以获得更好的远期镇痛效果，除此之外，灌注“鸡尾酒”注射治疗方法还拥有注射简单、节省手术时间等诸多优势。

Abstract：Objective To detect the clinical results of intraarticular cocktail application in TKA.Methods 102 patients underwent total knee arthroplasty at Beijing TsinghuaChanggung Hospital.52 patients in intraarticular injection group，11 were male and 41 were female，with an average age of (68.2±6.4)years old.50 patients in tissue injection group，including 16 males and 34 females，with an average age of (63.8±12.5)years old.The difference of visual analogue scale (VAS)，hemoglobin (Hb) and Albumin(Alb) between two groupswere analyzed.Results There was no difference in Hb and Alb between two groups，otherwise，the VAS of 24 hours postoperative in intraarticular injection group was significant lower than tissue injection group.Conclusion Although intraarticular cocktail injection can not give a significantly advantage to traditional method，but it can obviously reduce pain in long.In addition，intraarticular cocktail injection has many advantages such as simple injection and saving operation time compared with tissue injection after TKA.
